Traitement institutionnel et processus psychanalytique : le travail du psychanalyste à l’écoute du matériel institutionnel

Résumé — Se basant sur son expérience institutionnelle de psychanalyste travaillant avec une équipe de santé mentale, l’auteur cherche à décrire le travail psychique du psychanalyste dans le cadre du dispositif thérapeutique qu’elle a contribué à créer en utilisant les outils conceptuels de la psychanalyse pour les appliquer au traitement institutionnel. À partir d’un moment dynamique de la cure d’un adolescent psychotique traité en hôpital de jour au travers d’une séquence de jeu dans un atelier thérapeutique, elle tente de montrer comment elle utilise le modèle de la supervision pour repérer, comprendre et élaborer en équipe les contenus latents et les phénomènes transférentiels qui se jouent dans cet espace de soin. Grâce à cette élaboration interprétative faite à plusieurs, il devient possible de dégager un processus psychothérapique analogue au processus de la cure.Mots-clés : Élaboration interprétative, Transfert, Interprétation, Traitement institutionnel, Processus, Travail d’équipe, Perlaboration, Institution. Summary — On the basis of her experience of institutions in so far as a psychoanalyst working with a team in mental health, the author attempts to describe the psychic work of the psychoanalyst in the context of a therapeutic framework which she has contributed to creating, with recourse to the conceptual tools of psychoanalysis, in order to apply them to institutional treatment. With reference to a dynamic moment in the treatment of a psychotic adolescent patient being treated in a day care hospital, and in the context of a episode of play in a therapeutic workshop, she attempts to show how she uses the model of supervision in order, with her team, to locate, understand and elaborate the latent content and transferential phenomena at work in this establishment. Thanks to the group’s interpretative elaboration, a psychotherapeutic process analogous to the process of treatment can be identified.Keywords : Interpretative elaboration, Transference, Interpretation, Institutional treatment, Process, Teamwork, Working through. Zusammenfassung — Indem sie sich auf ihre institutionelle Erfahrung als Psychoanalytikerin, welche mit einer Equipe arbeitet, stützt, sucht die Autorin, die psychische Arbeit des Psychoanalytikers zu beschreiben, im Rahmen des therapeutischen Dispositivs, welches sie mitgeholfen hat, es aufzubauen. Von einem dynamischen Moment in der Kur eines psychotischen Adoleszenten ausgehend, im Rahmen eines Tagesspitals und anhand einer Spielsequenz in einem therapeutischen Atelier, versucht die Autorin, aufzuzeigen, wie sie das Modell der Supervision benützt, um zu erfassen, zu verstehen und, mit ihrer Equipe, die latenten Inhalte und die Übertragungsphänomene, die sich in diesem Pflegeraum abspielen, auszuarbeiten. Resumen — Basá ndose en su experiencia institucional de psicoanalista que trabaja con un equipo de salud mental, la autora va a describir el trabajo psíquico del psicoanalista en el marco del dispositivo terapéutico que ella ha contribuido a crear utilizando las herramientas conceptuales del psicoaná lisis para aplicarlas al tratamiento institucional. Partiendo de un movimiento diná mico de la cura de un adolescente sicótico tratado en el hospital de día a través de una secuencia de juego en un taller terapéutico, ella trata de mostrar como ha utilizado el modelo de supervisión para localizar, comprender y elaborar en equipo los contenidos latentes y los fenómenos transferenciales que actúan en este á mbito de cura. Riassunto — Basandosi sulla propria esperienza istituzionale in quanto psicoanalista che lavora con un’équipe di salute mentale, l’autrice cerca di descrivere il lavoro psichico dello psicoanalista nell’ambito del dispositivo terapeutico che ha contribuito a creare utilizzando gli strumenti concettuali della psicoanalisi per applicarli alla cura nelle istituzioni. A partire da un momento dinamico della cura di un adolescente psicotico curato in ospedale di giorno attraverso una sequenza di gioco in un gruppo terapeutico, tenta di mostrare come usa il modello della supervisione per individuare, capire ed elaborare con l’équipe i contenuti latenti e i fenomeni transferali presenti in questo spazio di cura.
