Entre conversion et somatisation : la relation métonymique

À partir d’un cas clinique situé à la limite de la névrose hystérique et d’une organisation psychique insuffisamment mentalisée, le texte montre l’accordage du cadre psychothérapique et des fluctuations du fonctionnement mental de la patiente. Une cure de relaxation a rendu possible un travail psychique plus fluide, permettant le glissement de certaines somatisations s’apparentant à un processus métonymique vers la reconnaissance de conflits psychiques ici assimilés à des métaphores.Mots-clés : Conversion/métaphore, Somatisation/métonymie, Relaxation. Through a clinical case bordering on hysterical neurosis and an insufficiently mentalized psychical organization, the text shows the attunement between the psychotherapeutic setting and the fluctuation of mental functioning in the patient. A cure of relaxation made more fluid psychical work possible. This enabled certain somatizations related to a metonymic process to give way, leading towards being able to recognize psychical conflicts, in this case assimilated with metaphors.Keywords : Conversion/metaphor, Somatization/metonymy, Relaxation. Von einem Einzelfall an der Grenze der hysterischen Neurose und einer ungenügend vergeistigten psychischen Organisation ausgehend, zeigt der Text die Verbindung vom psychotherapeutischen Rahmen und den Schwankungen des geistigen Funktionierens der Patientin. Dank einer Relaxationskur wurde eine flüssigere psychische Arbeit ermöglicht, wodurch einige Somatisierungen, die einem metonymischen Vorgang verwandt waren, in die Anerkennung psychischer Konflikte, welche hier Metaphern gleichgesetzt werden, übergingen.Schlagwörter : Konversion/Metapher, Somatisierung/Metonymie, Relaxation. A partir de un caso clínico limítrofe entre una neurosis histérica y una organización psíquica insuficientemente mentalizada, el texto muestra la concordancia del marco psicoterapeutico y de las fluctuaciones del funcionamiento mental del paciente. Una cura de relajación ha permitido un trabajo psíquico más fluido, permitiendo el deslizamiento de ciertas somatizaciones emparentandose a un proceso metonímico hacia el reconocimiento de conflictos psíquicos que se han asimilado a metáforas.Palabras claves : Conversión/metáfora, Somatización/metonímia, Relajación.
