Risque d’évolution psychopathique chez une enfant agitée

À travers l’exposé d’un cas clinique d’une enfant agitée à risque d’évolution psychopathique, sont dégagées quelques particularités du fonctionnement psychique d’enfants présentant d’importants troubles du comportement, placés en institution spécialisée. D’intelligence normale, ils sont rassemblés à partir de leurs symptômes visibles (agitation extrême, manifestations comportementales bruyantes, passages à l’acte auto et hétéro-agressifs, violences), symptômes fortement perturbant pour eux-mêmes et pour l’environnement au sens large puisque pouvant aboutir à un rejet scolaire, familial et social. La focalisation sur des comportements manifestes et dérangeants ne rend pas compte d’un diagnostic précis, lié à une entité clinique et psychopathologique univoque. Les cliniciens d’orientation psychanalytique s’attachent, eux, au repérage de l’organisation psychopathologique au sein de laquelle le symptôme prend place, fonction et sens. Les observations cliniques au cours du placement et la méthodologie projective permettent de repérer les différents secteurs de fonctionnement qui coexistent au sein de la psyché et dont les prépondérances respectives orientent le mode de fonctionnement et le diagnostic. Dans une perspective de psychopathologie psychanalytique, l’organisation psychopathologie est ici analysée à travers le prisme du complexe d’Œdipe dont les différents points constituent des axes d’observation du fonctionnement psychique global. By way of of a clinical case presentation of an agitated child at risk of psychopathological evolution, certain particularities in the psychic functioning of children presenting important behavioral troubles and placed in specialized institutions are brought to light. Possessing normal intelligence, they are put together because of their visible symptoms (extreme agitation, noisy behavioral manifestations, self- and other- destructive acting out, violence), symptoms which are very disturbing for them and for their global environment in that there is a risk of expulsion from school, family and the social network. The focalization on their manifest, disturbing behaviors does not determine a precise diagnosis, linked to an unequivocal, clinical and psychopathological entity. Clinicians with a psychoanalytical orientation attempt to uncover the psychopathological organization at the heart of which the symptom takes its place, its function and meaning. The clinical observations during placement and tools from projective methodology make it possible to discover the different sectors of functioning which coexist at the heart of their psyche and whose respective importance orients the functioning mode and the diagnosis. In a psychopathological, psychoanalytical perspective, the psychopathological organization is analysed here through the intricate prism of the Œdipal complex whose different points constitute the axes of observation of global psychic functioning. La exposición del caso clínico de una niña agitada con riesgo de evolución psicopá tica pone de relieve algunas particularidades del funcionamiento psíquico de niños con importantes trastornos del comportamiento internados en instituciones especializadas. De inteligencia normal, se los ha reunido a partir de sus síntomas visibles (agitación extrema, manifestaciones de comportamiento ruidosas, actuaciones auto y hetero agresivas, violencia) síntomas altamente perturbadores para ellos mismos y para el entorno en sentido amplio, pudiendo desembocar en rechazo escolar, familiar y social. La focalización en los comportamientos manifiestos y molestos no da cuenta de un diagnóstico preciso, relacionado con una entidad crítica y psicopatológica unívoca. Los clínicos de orientación psicoanalítica se centran en explorar la organización psicopatológica en la que aparece el síntoma, la función y el sentido. Las observaciones clínicas en la institución y la metodología proyectiva permiten localizar los distintos sectores de funcionamiento que coexisten en la psiqué y cuyas preponderancias respectivas orientan el modo de funcionamiento y el diagnóstico. En una perspectiva de psicopatología psicoanalítica, la organización psicopatológica se analiza en este caso bajo el enfoque del complejo de Edipo : sus distintos aspectos constituyen los ejes de observación del funcionamiento psíquico global.
