Niveaux de fonctionnement mental et psychothérapie psychanalytique

L’évaluation de la qualité du fonctionnement mental d’un patient est sans doute l’élément le plus déterminant dans le choix du type d’interventions thérapeutiques et des aménagements techniques du cadre analytique. Face à un fonctionnement névrotique, le psychothérapeute s’emploie à favoriser chez son patient l’émergence et l’appropriation de contenus symbolisés préalablement refoulés ; l’instrument optimal pour ce travail d’exploration et de découverte est le dispositif psychanalytique classique. Avec un fonctionnement non névrotique, le thérapeute tente de faciliter chez son patient la transformation de contenus non symbolisés ; il doit alors réduire la teneur en symbolique de la situation analytique pour « rencontrer » ces éléments non symboliques, soit par des aménagements du cadre analytique, soit par l’adoption d’une attitude « malléable » et activement soutenante.Mots-clés : psychothérapie psychanalytique, fonctionnement mental, symbolisation, mentalisation, psychothérapie de soutien, aménagements du cadre analytique, eléments non symboliques de la communication, personnalités primitives. Assessment of the quality of a patient’s mental functioning is undoubtedly the determining factor in the selection of therapeutic interventions and the technical modifications brought to the analytic setting. When dealing with neurotic functioning, the therapist attempts to facilitate the emergence and appropriation of symbolized repressed contents in the patient ; the optimal instrument for this work of exploration and discovery is the classical psychoanalytic setting. When confronted with non-neurotic functioning, the therapist strives to promote the transformation of non-symbolized contents in his patient ; he must then reduce the ratio of symbolic characteristics of the analytic situation to « meet » those non symbolic aspects of the patient’s functioning, by modifying the analytic setting or by adopting a malleable and supportive attitude.Keywords : psychoanalytic psychotherapy, mental functioning, symbolization, mentalisation, supportive psychotherapy, modifications of the analytic setting, non-symbolic elements of communication, primitive personalities.
